DESCRIPTION:
The Community Health Worker - Case Manager, located in the Support Services Department, is a member of the Maternity Support Services (MSS) team, which includes the Coordinator, Medical Providers, Social Worker, Nutritionist, Public Health Nurse, Community Health Worker, and WIC staff.  This position works one-on-one with eligible clients and their family in identifying needs and providing case management activities throughout the pregnancy and up to two months postpartum. He/she must have a working understanding of the First Steps Program in order to provide the required case management activities.

 QUALIFICATIONS:

1. An associate’s degree in behavioral science or related field and two years’ of closely related work experience.
2. Bilingual and bicultural in English/Spanish
3. Possess a valid Washington State driver’s license.
