Eligibility for benefits begins the first of the month following 90 days of employment, or by contractual agreement.
Medical Benefits
Medical Plan
CBHA provides a PPO plan through First Choice Health Administrators. Employees are responsible for any additional premiums required for family coverage.
Dental Plan
The clinic contributes $1,200 a year for dental services provided by our dental staff at either Othello Family Dental or Wahluke Family Dental.
Vision Plan
Our benefits cover one standard eye exam per year and as much as $200 per year in hardware.
Life Insurance/ Long Term Disability
Life Insurance
Our life and accidental coverage pays $50,000 for hourly employees and $100,000 for salaried employees. Supplemental life is also available for a low rate.
Long Term Disability
CBHA provides long term disability coverage through Lincoln Financial. It entitles an employee to a 60% monthly benefit of up to $10,000, following six months of disability.
Profit Sharing/ 403(b) Plan
Profit Sharing/ 403(b) Savings Plan
As a financially secure organization, CBHA is able to offer a profit sharing plan to help employees achieve financial stability for retirement. Employees are eligible to enroll in the profit sharing plan following one year of service. Once enrolled, an employee is entitled to an annual profit sharing distribution, provided he or she has worked at least 1,000 hours during the preceding calendar year. CBHA contributes a percentage of the employee's annual earnings as determined annually based upon the association's financial ability. In past years, employer contributions have totaled 8%. Employees are fully vested after five years of employment.
Employees can also make voluntary contributions to the 403(b) or Roth 403(b) plan, subject to IRS limitations. Employees contributing to the 403(b) plans are fully vested upon entry.
Employee Assistance
Employee Assistance Program
All CBHA employees and their dependents are able to utilize the Employee Assistance Program (EAP) through First Choice Health, which offers a wide range of services to solve concerns and problems including marital and family issues; emotional concerns such as depression and anxiety; problems with substance abuse and gambling; and balancing the demands of work and home, including legal issues, financial concerns, personal/family mediation, child and eldercare services. EAP services are completely confidential and are available 24-hours a day, 7 days a week.
Loan Repayment Opportunities

Other Benefits
Paid Time Off
To allow the maximum amount of flexibilityfor the employees, all paid time off - including time for vacation and illness - is accrued as personal leave. Salaried employees accrue at the rate of 24 days per year. Hourly employees accrue at the rate of 8 days per year for the first year, 18 days the second year, and 21 days per year in the third and fourth years, and 24 days per year beginning with the fifth year. Personal leave may be accumulated from year to year with no maximum cap. Unused personal leave will be paid to employees upon separation, based on personnel policy guidelines. Thirty-six (36) hours of personal leave must be maintained in the personal leave bank at all times.

The following holidays are observed and paid by CBHA:

Martin Luther King Day and President's Day are considered floating holidays and are used at employee's discretion.
Continuing Education
Medical and dental providers, as well as qualified contracted staff, are eligible for 40 hours of educational leave. The allotted dollar amount for continuing professional education (CPE) is based on contractual negotiations.
Dues and Licenses
CBHA will pay for one each of local, state, and national memberships. CBHA will pay for Washington State licensure renewals and Drug Enforcement Administration registration renewals.
Employees Fitness Facility
CBHA staff have access to the newest fitness equipment including treadmill, elliptical, bicycles and weights. The fitness room includes men's and women's lockers, showers and changing areas. Staff can sign-up to meet with an on-staff trainer to assist in their fitness goals.
