Health Insurance: Employer pays 100% of the Employee premium and as follows for the Dependent premium:

Employee & 2+ Children-$70 per pay period
Employee & Spouse- $65 per pay period
Employee, Spouse & 1 Child- $110 per pay period
Employee, Spouse & 2+ Children- $135 per pay period
PPO with First Choice. $300 Deductible, $900 per family. $2,000 Out of Pocket Maximum, $6,000 per family. 80% Preferred Provider, 60% Non-Preferred Provider.
Dental: $20 copay and $1,200 annual maximum for services provided at CBHA dental clinic.
Vision: One eye exam every 12 months. Frames, lenses and contacts up to $200 every 12 months.
Life Insurance: All full time providers receive maximum of $100,000 paid by the Association. Supplemental insurance is also available.
*Long Term Disability: Income protection benefit pays 60% of monthly salary (maximum $10,000/month) for disability of three months or longer.
Profit Sharing/Pension Plan: CBHA has a Profit Sharing Plan to help employees achieve financial stability in their retirement. CBHA will contribute 8% (3% of which is based on the financial ability of the association; past years have shown 8%) of total salary for each employee during each year in which the employee meets eligibility for the Profit Sharing Plan. (Eligibility is defined as maintaining full-time status and 64 paid hours per pay period). Employees vest after 5 years. There is no required employee match.
403(b) plan: All full time providers may contribute to their own funds from date of hire subject IRS limitations. Employees are fully vested in plan upon entry.
Payroll Direct Deposit: Every other Friday (26 times a year) to any financial institution in the United States.
Paid Time Off: To allow the maximum amount of flexibility for the employees, all paid time off, including time for vacation and times of illness, is accrued as Personal Leave. All full time providers accrue at the rate of 24 days per year. Personal Leave may be accumulated from year to year, with no maximum cap.
Holidays: The following holidays are observed and paid by CBHA;

Martin Luther King’s Birthday and President’s Day are considered floating holidays and are used at employee’s discretion.
Continuing Education: Medical and Dental Providers, and eligible contracted staff are eligible for 40 hours of educational leave. The allotted dollar amount for CPE is based on contractual negotiations.
Dues and Licenses: CBHA will pay for one local, state and national membership. CBHA will pay for Washington State licensure renewals and DEA registration renewals.
